Utah requires businesses with employees to register for a withholding tax account. Here is a link to the online registration process:

Enter information for each officer, general partner, managing member, trustee or enterprise owner. The Social Security Number is required (Tax Commission Rule R861-1A-15). The Tax Commission will only discuss account information with the officer(s) or owner(s) listed. You are required to notify the Tax Commission of any changes to the officer(s)/owner(s). You will be asked if you have additional officer’s or owner’s in the next screen.

- Any person (including employees, corporate directors, corporate officers, etc.) with the authority to direct accounting processes or who is required to collect, account for and pay any taxes and fails to do so will be liable for a penalty equal to the total amount of tax not collected, not accounted for or not paid (Utah Code §59-1-302).
