Form 1095-C:
- This is a form that employers must provide to their employees as an annual statement of coverage.
- It is officially the Employer-Provided Health Insurance Offer and Coverage Form.
- Employers that are ALE’s are required to provide this form to their employees by January 31st following the end of the previous calendar year.
- When previewing the employee 1095-C Form, you can easily navigate to the next employee or select the previous employee.
- The form contains the basic structure but is not intended to be printed and provided directly to the employee.
- Updates can be made directly to the form to adjust an Offer of Coverage code (box 14), Employee Share of Lowest Cost Monthly Premium (box 15) or Safe Harbor code (box 16).
